In accordance with Texas House Bill 4224 and the rules of the Texas Board of Chiropractic Examiners (TBCE), this notice provides mandatory information regarding your rights to access health care records, licensing authorities, and the process for filing consumer complaints.
1. How to Request Your Health Care Records
To request a copy of your chiropractic or billing records, please follow the procedure below:
Submission: All requests must be submitted in writing, signed, and dated by the patient or legal guardian. Please include the patient's full name, date of birth, and the specific dates of service requested.

Processing Time & Fees: In compliance with Texas law, the clinic will respond to your request within 15 business days. Reasonable, cost-based fees may apply for duplicating records or providing digital summaries.

If you have a complaint regarding the care received, professional conduct, or consumer rights violations, you may contact the following state authorities:
For Professional Conduct & Care Complaints: File a formal complaint directly with the TBCE. Anonymous complaints are not accepted by law. You can submit complaints online via the {Link: TBCE Complaint Portal https://texas.gov}.
For Deceptive Practices, Billing, or Privacy Violations: File a consumer complaint with the Consumer Protection Division of the Office of the Texas Attorney General. Submissions can be completed online at the {Link: Texas Attorney General Website https://texasattorneygeneral.gov}.
