Medical Records and Complaints

1. Requesting Your Health Care Records

Patients may request copies of their health care records from RAPHA Chiropractic & Acupuncture, PLLC.

To request your records, submit a written, signed request including your name, date of birth, contact information, the records or dates of service requested, and where you would like the records sent.

We may verify your identity before releasing records. Applicable fees may apply as permitted by law.

If RAPHA uses an electronic health records system capable of fulfilling the request, Texas law generally requires an electronic health record requested in writing to be provided within 15 business days. Tex. Health & Safety Code § 181.102.

2. Licensing and Disciplinary Authority

RAPHA Chiropractic & Acupuncture, PLLC provides chiropractic services through a chiropractor licensed by the Texas Board of Chiropractic Examiners (TBCE).

Patients may contact the TBCE regarding licensing, professional conduct, or disciplinary matters.

3. Filing a Consumer Complaint

Patients may file a complaint concerning chiropractic care, professional conduct, or applicable health information privacy matters with the appropriate regulatory authority.
